Clinical and radiological findings: A 23-year-old male sustained a 30-foot fall resulting in a 3a open fracture on the lateral side of the distal tibia, classified as a B type pilon fracture. Initial management included debridement, irrigation, limited articular fixation through the traumatic wound, and application of an external fixator with primary closure of the open wound. Radiological evaluation confirmed significant articular involvement with a Volkmann fragment.
Planning remarks: The definitive surgical plan involved a staged approach. The initial approach was posterolateral to address the Volkmann fragment and fibula, followed by an anteromedial approach for the main articular impaction and comminution. Fragment-specific buttress plating was planned for stabilization.
